Information Regarding 2008-2009
Taylor University is pleased to continue the Student Accident and Sickness Insurance Plan especially designed for Taylor students and their dependents. All Taylor University Students are required to carry some form of health insurance. If you currently have health insurance and believe it is adequate, you may waive participation in the University sponsored plan.

• Expiration date: August 14, 2009
• Cost: $174.50 per semester for US students
• Cost: $240.50 per semester for International students
• 1st Semester coverage dates: August 15, 2008 through February 1, 2009
• 2nd Semester coverage dates: February 2, 2009 through August 14, 2009
• Policy maximum amount for US students: $10,000.00 per sickness or injury
• International Students policy maximum amount is: $20,000.00 per sickness or injury

Gallagher Koster was selected to manage the Student Accident and Sickness Insurance Plan at Taylor University for the 2008-2009 plan year. If you believe you currently have adequate coverage and would like to waive the Student Accident and Sickness Insurance Plan, log onto: www.gallagherkoster.com, click on Student Access, and select Taylor University from the drop down menu. First time users will be required to create a unique User Account by entering a user name, password, first and last name, Student ID number, date of birth and email address. Once created, log in and click on ‘Student Waive/Enroll Forms' and select the ‘2008-2009 Taylor University Waiver Form'. If waiving the insurance, have your current health insurance ID card ready as you will need this information in order to complete the waiver form.
The annual deadline to submit an insurance waiver is September 7, 2008. For new students enrolling in the spring the deadline is February 8, 2009. This is an ONLINE process only. Paper submissions, letters, phone call or faxes will not be accepted.
Intercollegiate Athletics:
Taylor University provides additional excess insurance coverage for students who participate in intercollegiate athletics at no additional cost to the student. Since the Student Insurance Plan provides base coverage for the first $1,500 of covered expenses, all students who participate in intercollegiate athletics are strongly encouraged to enroll in the Student Insurance Program. In the event of a claim, the first $1,500 of covered expenses will be paid by the Student Insurance Plan, which is primary coverage. Then the family or employer group coverage, which is secondary, must contribute to its maximum and then the Taylor University Athletic Insurance policy will pay for certain ‘Usual and Customary' medical expenses remaining to the maximum stated. If there is no other insurance available, the Athletic Insurance policy will pay ‘Usual and Customary' medical expenses remaining to the maximum stated. All athletes must provide information about their personal coverage. A request for this information will be distributed by the Athletic Department.
